PERMIT APPLICATION REQUIREMENTS

The Building Permit Application Form is universal.  This form is used for all types of work and requests.  It will become obvious to you that certain questions or “blanks”, don’t apply to what you are doing.  This is understandable and expected.  Fill in only the information that applies to what you are doing.  If you have questions, please stop in to our office or call 978-356-6605 x8.

EVERY application package, regardless of work, MUST include:

  1. Completed Building Permit Application Form or Amended Building Permit Application Form with all appropriate information supplied AND signed by owner or agent of owner.
  2. Workers' Compensation Affidavit with Certificate of Insurance or Insurance Affidavit from Contractors or Insurance Affidavit from Homeowners.
  3. Debris Disposal Affidavit completed and signed.
  4. Homeowners License Exemption Form  (only needed when homeowners are Performing Work Themselves or are Signing Off As The General Contractor) completed and signed.
  5. A check for the exact amount of the permit fee made payable to the Town of Ipswich.
